Something I'd like to see in the First-Aid system:

Holding direct pressure on wounds to slow bleeding, for scenarios where bandages are not immediately available. Bandage, sutures, or tourniquets are obviously preferable, so I think there should still be drawbacks to simple pressure. Other notes and ideas regarding this:
- It uses up one or both, (maybe depending on bleeding severity), of your hands so you can't properly swing a long weapon or spear.
- Increased infection risk. Unless you just so happen to be wearing a sterile pair of nitrile gloves, you risk bacteria and other nastiness entering your blood stream.
- Unable to apply direct pressure with hemophobic trait.
- Rate of bleeding reduction is affected by first aid skill.
- Bindable to a hotkey, for those instances of a neck laceration that can sometimes kill you faster than you can tear up your shirt. Might even buy you enough time to improvise a bandage, (if you still have a free hand), or run into a house, check a trunk for first aid kits, etc.

I'd also like to see more complex stuff like arterial bleeding vs venous bleeding, injuries to major arteries requiring special attention (tourniquets) or even surgical intervention, internal bleeding, occlusive wound dressing for puncture wounds to the chest cavity... But that might be a little overly complex to keep balanced and fun for gameplay purposes.

Simple direct pressure feels like it would fit right into the PZ world though. Plus it's real world stuff, pressure's the first thing you do with just about any major bleed.

Regarding the no time to tear up shirt, it is why most folks carry bandages, ripped sheets(sterilized ) in fanny packs or in their main inventory. (Once they have the space, equipment etc.)

Now to actually discuss your suggestion:
It really does makes sense and fits the PZ setting.

What is though the thing that is most important regarding the game how much extra annoyance(for the lack of better word,) or extra work it adds to the game. Don't get me wrong, i mean if this system was implemented. You would need another extra pair of gloves(the items at least exist in the game), you would need to have one of your hands to provide pressure....and so on.

->Which may hinder you quite a lot. Cuz this way ripping clothes to get rags would take more time, unless for instance you are standing(sitting) still. Or you cannot use one of your hand, or in the unlucky situation your leg.
-> Also you should take into account, that often it is not just wound you have gained. What would you do if you have multiple wounds? ->It would work for multiplayer, where your pals could also hold you down while your medic treats you. But in single player it is probably more of an extra layer of annoyance then how much it is worth it.

About the further suggestions i think whereas it would add to the PZ complexity and realism it would make the game even more punishing, and for many folks especially newer guys, probably give even more cases where they just rage quit, and might post something not nice on the forum.
+ The dev team would need to make quite some time to research all those different bleedins, injuries etc. to be able to decide how to implement it. Or hire someone with the knowledge as an advisor.

For instance me personally am more interested in implementing an actual electricity system, with electrician knowledge requirements...but even that would be a bit too much, even if you could do your own wiring (like in Barotrauma). But probably this extra completexity would just be a bit too much.

Good points. It's not something meant to fix all your bleeding troubles though, it's more something you'd do out of desperation. If you've got multiple wounds, you'd pick the worst one and try to stabilize it, let the others bleed and hope you can improvise something before you bleed out. Maybe you've used all your bandage supply, or just plain forgotten to restock your backpack. Accidents happen but often there's a method to work around them, and moments like that are my favorite parts of the game. This is just another method to narrowly escape death, (or not escape death.)

But yeah regarding dev workload I'd definitely rather them finish their current roadmap stuff before really putting the time into a first aid overhaul. We could use one, but I think people are more anxious for the new crafting system, animals, and NPC's.

I love this idea, I think it would well in PZ with the possibility of removing some of the difficulties brought forward by Nordil. My opinion based on both :
- For simplicity it would probably be best to just use one hand for all, and for multiple wounds it could be based on severity and location. (Example, all neck wounds > all head wounds > deep wounds on legs, arms, torso > lacerations on legs, arms, torso > scratches on legs, arms torso). This would allow it to be binded to a hotkey and also allow for the increased infection chance.
- On top of the hotkey, as any other medical action can be done make it possible to hold down a specific part in the health menu, this would allow the multiplayer capability but also allow solo players to choose their own medical priority. The pressure system could be implemented on top of the medical system without changing how it works so if it goes unused by those who don't like it, there is no negative and they don't have to carry the gloves (though realistically if you're putting gloves on first you mine as well just put the bandage on).
- Perhaps it can slow bleeding by 50% at 0 first aid skill, but as a hemophobe only 25% to simulate indirect pressure. Have bleed slowdown go up about 2% per first aid level and you'll end up with 70% slowdown or 45% with hemophobe at level 10. Devs could balance it over time or when/if they do a first aid rework, but honestly this might make a good mod in the meantime if somebody could figure out how to do it.
- It would absolutely take up a hand to do this, of course, zomboid already has a system for using two-handed weapons with only one, it could be set to trigger when holding pressure on a wound as well, animations can come later. If the medical system has no new debuffs for not using hand pressure, it remains a complete choice for those who don't want to take a little more time for ripping a bandage or something. Tbh though if it's between death and a slower ripping of cloth, I'll take the slower ripping.

Perhaps cauterizing wounds would be a bit easier to implement?
Since we already have fire, and even some stainless steel items, lotsa metal stuff?
It could "speed up" recovery for certain wounds, but it would give you a longer lasting pain debuff?
(this actually just came to my mind while rereading the whole topic. I am always down to reasonable suggestions to be debated:D)

Yeah I didn't consider the hotkey thing with multiple wounds. I think ultimately best option would be to open your health panel and right click then select "hold pressure", like you would with any other medical action. If you were to hot key it, even with a severity system, you'd probably end up auto-pressuring a wound you didn't want to. Hotkey may be more trouble than it's worth

That's why it would be a good idea to put both imo, as the hotkey (which can be removed in controls) would be an emergency "oh s***" button but could be overwritten in the health panel by choosing a different wound in there.

Could be, i just racked my brain a bit while thinking through OP-s suggestion. (It is one of those rare threads where something new comes up that actually really fits the setting and the narrative. However implementation might have its limits so perhaps some other simpler solutions might be better. Allthough then we need to think how much we lose with the simpler solution in regards to the original one with narrative and setting fitting. Hard choices!:D)

Probably going with your added idea, the foraging could get a new item too of which a new kind of poultice, soothing poultice could be made. And since they devs are introducing all kinds of animals, perhaps we will get bugbites, mosquito bites and such as well, meaning that a soothing poultice could have use for other stuff, and not become just a single use item.
